Post Placement Verification Letter
Local police clearance on letterhead
Employment letter on letterhead (This can not be written by yourself personally. It must be written by employer or accountant.)
Photocopy of Passports
Powers of Attorney - must be typed
Statement of Living Conditions
Promise to Register with Consulate
Promise to Register with Post Placement
Copy of I-171H Notice of Favorable Determination, cabled to Moscow, Russia, DO NOT notarize the original, only copies.
ALL DOCUMENTS MUST BE LESS THAN 12 MONTHS OLD AT TIME OF SUBMISSION. Your notary's commission cannot expire within a 12 month period following the notarization of these documents. Some regions may require that police and employment documents be less than 3-6 months old. All medicals must be less than three months and will not be requested until a referral has been accepted as regions require different medical forms. Psychiatric evaluations may be necessary as well. EAC suggests you locate a psychiatrist in your area in the event that this is needed after the acceptance of a referral.

your files as you will need it for your Embassy Appointment in Russia)

13. Photo of Adoptive Parent(s)
14. Color copies of passports of adoptive parents – you will need 2 notarized, certified, and
apostilled for your dossier..
15. $450 Home Study Review Fee. Only applicable if EAC does NOT conduct your Home Study.
This fee is required to support Hague Home Study requirements.

***In order to complete your adoption in a timely manner, please complete these fifteen (15) Registration Documents (16 if applicable) and send to the EAC office as soon as possible. ***

After Registration Document completion, follow the Document Authentication List in the Foreign Dossier tab in order to complete the Foreign Dossier.
